A Medical Coder is someone who converts a doctor's prescription, diagnosis, or other medical document into internationally recognised codes. These are coded for the purpose of keeping accurate medical records. The golden rule of the healthcare billing and coding department is to not code anything that is not documented in the medical record. When a claim is filed, providers use clinical documentation to justify reimbursement to payers.
